Ontario has launched its 2026–2027 call for proposals to create and expand approximately 75 new interprofessional primary care teams, aiming to connect 500,000 more people to consistent, coordinated care. This initiative is part of a $2.1 billion investment through Ontario’s Primary Care Action Plan, with a goal to connect 100% of Ontarians to primary care by 2029.
Who can apply for the funding?
You’re eligible if you are:

A primary care provider or clinician in Ontario

Working in partnership with your Ontario Health Team (OHT) and Primary Care Network (PCN)

Part of a team-based model such as:

Family Health Teams
-
Community Health Centres
-
Nurse Practitioner-Led Clinics

Indigenous Primary Health Care Organizations

What should your proposal include?
To qualify for 2026–2027 funding, your proposal must:

Provide net new, ongoing attachment to primary care for underserved populations

Be ready to launch by Spring 2026 with infrastructure and staffing in place

Align with key primary care principles:

Connected to social supports
-
Culturally safe and inclusive
-
Digitally integrated
-
Timely and responsive

Designed in partnership with local communities

How much funding is available?
This is a multi-year funding opportunity, with scope and budget tied to your community’s needs and your proposal’s readiness. Up to two additional proposals per OHT may be accepted if they align with strategic priorities or refine a previous submission from 2025–2026.
