In many northern and remote communities across Canada, a medical trip can mean a full day on the road to reach care in Winnipeg, Edmonton, Saskatoon, or Thunder Bay.<\/span> <a href=\"https:\/\/www150.statcan.gc.ca\/n1\/daily-quotidien\/241104\/dq241104a-eng.htm\"><span style=\"font-weight: 400;\">Statistics Canada reported<\/span><\/a><span style=\"font-weight: 400;\"> that 18% of First Nations people living off reserve and 40% of Inuit travelled outside their community for healthcare in the past year. That travel can affect Elders, patients, families, and staff. It can also create a bigger gap. I\u2019ve heard it from Indigenous, First Nations, Metis, and Inuit communities across northeran Alberta and seen the same kind of concern in remote parts of BC, where mountain roads, <\/span><a href=\"https:\/\/www.movemobility.ca\/fr\/ressources\/winter-safety-for-first-nations\/\"><span style=\"font-weight: 400;\">winter weather<\/span><\/a><span style=\"font-weight: 400;\">, and long travel times can make an already hard trip even harder. I\u2019ve also seen this thinking in Ontario, where some communities are trying to figure out how to keep <\/span><a href=\"https:\/\/www.movemobility.ca\/fr\/ressources\/importance-community-shuttle\/\"><span style=\"font-weight: 400;\">local transportation<\/span><\/a><span style=\"font-weight: 400;\"> available while still helping people get to care farther away.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The common problem is usually the same.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A community has a vehicle that helps people get around locally. Then that same vehicle gets used for a long medical trip. Once it leaves, it\u2019s out of service for everyone else until it comes back.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s where the pressure starts.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Let\u2019s say someone needs to travel from the Fort McMurray region to Edmonton for care. That can mean about four hours each way. Add in the appointment, road conditions, fuel stops, and a few extra errands that often come with a trip into the city, and suddenly that one vehicle is tied up for most of the day.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Meanwhile, local needs don\u2019t stop.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">An Elder may still need a ride to the health centre. Someone else may need help getting to a local appointment. A person without a personal vehicle may be waiting on that same van to get basic care. If the vehicle is already gone, those people may be left waiting, rescheduling, or trying to find another option.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s why this issue is bigger than distance alone. That trip is important.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But what gets missed is what happens back in the community while that vehicle is away.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">A local van may be used for several things in one day, including:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Health centre visits:<\/b><span style=\"font-weight: 400;\"> Short rides to local appointments.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Elder transportation:<\/b><span style=\"font-weight: 400;\"> Safe and reliable trips for Elders.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Accessible transportation:<\/b><span style=\"font-weight: 400;\"> Support for people using wheelchairs, walkers, or canes.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Family support:<\/b><span style=\"font-weight: 400;\"> Rides for people who don\u2019t have a vehicle of their own.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Community transportation needs:<\/b><span style=\"font-weight: 400;\"> Day-to-day travel that helps people stay connected to care.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">When one long trip takes that van out of the schedule, the whole local plan can shift.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Staff may have to move appointments around. Families may have to step in and find another ride. Some people may simply have to wait until the vehicle returns. That can turn a transportation issue into a healthcare access issue pretty fast.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This is one reason some communities are taking a closer look at<\/span><a href=\"https:\/\/www.movemobility.ca\/fr\/ressources\/services-de-transport-de-patients\/\"> <span style=\"font-weight: 400;\">services de transport de patients<\/span><\/a><span style=\"font-weight: 400;\"> and asking if their current setup is putting too much strain on one vehicle.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To me, that\u2019s the core question.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s not just, \u201cCan we get one person to care?\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s also, \u201cWhat happens to everyone else while that trip is happening?\u201d<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3>Why does local transportation feel stretched when one vehicle does everything?<\/h3>\n<p><span style=\"font-weight: 400;\">In many communities, <\/span><a href=\"https:\/\/www.movemobility.ca\/fr\/ressources\/dilemme-des-organisations-a-but-non-lucratif-avec-des-vehicules-accessibles\/\"><span style=\"font-weight: 400;\">transportation resources<\/span><\/a><span style=\"font-weight: 400;\"> are already limited. There may be one main vehicle doing a lot of the work. There may be a small team managing rides. Additionally, there may also be a long list of people who rely on that transportation every week.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So when long-distance medical transportation gets added into the mix, the schedule can get stretched pretty quickly.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I\u2019ve seen that concern come up in places where the roads are long and flat, and I\u2019ve seen it in areas where the roads wind through mountains or face winter closures. The geography looks different, but the strain feels very similar.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Here\u2019s a simple example.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A community van is booked for a long trip out of town. At the same time, an Elder needs to get to the local clinic, and another community member needs a ride for a follow-up appointment in the community. The team now has to choose how to juggle those needs with one vehicle already gone.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s a hard spot to be in.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Nobody wants to feel like helping one person means another person has to wait. But when one van is expected to handle both local and long-distance medical transportation, that\u2019s often the reality.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s also why some teams start asking if there\u2019s a better way to protect local transportation access. They\u2019re looking at what one trip takes away from local access, and they\u2019re asking how to keep transportation working for the whole community, not just for one route on one day.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2>How can long-distance medical transportation wear people down?<\/h2>\n<p><span style=\"font-weight: 400;\">Long-distance medical transportation can be hard on anyone. But for people with <\/span><a href=\"https:\/\/www.movemobility.ca\/fr\/ressources\/wheelchair-accessible-vehicle-solutions\/\"><span style=\"font-weight: 400;\">probl\u00e8mes de mobilit\u00e9<\/span><\/a><span style=\"font-weight: 400;\">, it can feel even harder when the only available option is a personal vehicle.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I\u2019ve heard this come up in conversations with communities. Sometimes, when a community vehicle isn\u2019t available, people rely on family vehicles, staff vehicles, or whatever ride they can find. That may get someone to the appointment, but it doesn\u2019t always make the trip safe, comfortable, or dignified.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Picture someone who uses a wheelchair <\/span><a href=\"https:\/\/www.cma.ca\/healthcare-for-real\/does-where-i-live-affect-my-health-care\"><span style=\"font-weight: 400;\">travelling four hours<\/span><\/a><span style=\"font-weight: 400;\"> to care, then four hours back. If they have to transfer into a cramped car, that can mean sitting in a position that doesn\u2019t support their body well. Their wheelchair may be folded up or stored in the back. They may have limited room to move. They may not be able to stretch, adjust, or rest in a way that feels comfortable.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s a long time to feel stuck.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">And if that person is travelling for dialysis, surgery follow-up, or a specialist appointment, they may already be tired before the ride even starts. It\u2019s about more than getting from one place to another. It should give people enough space to sit comfortably, travel safely, and avoid painful transfers when possible.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For a person who uses a wheelchair, that may mean staying in their wheelchair during the ride instead of transferring into a cramped seat.
